Your initial appointment will consist of a discussion of your concerns and goals for treatment. The dentist and hygienist will evaluate your teeth, gums, joints, and perform an oral cancer screening. They will discuss the findings with you and come up with a treatment plan to address your needs and desires. Occasionally, treatment can be completed at the initial visit. However, a complex medical history or treatment plan will require an additional appointment. We want your treatment to be thorough and never rushed.
Please assist us by providing the following information at the time of your consultation:
- Online patient forms prior to the visit. Click here
- Any X-rays if applicable. Click here for a release form
- A list of medications you are presently taking.
- If you have dental insurance, please provide that information on the online forms and bring your card with you to your appointment.
Important
A parent or guardian must accompany all patients under 18 at the initial visit.
Please alert the office if you have a medical condition that may be of concern prior to your initial visit. Replacement joints, hospitalizations, recent surgeries, cancer treatment and blood thinning medications may require us to get clearance from your medical doctor before we perform treatment.
X-Rays
If your previous dentist has taken recent X-rays, you may request that they forward them to our office. Please click here for our Records Release Request form. We will take additional X-rays as necessary.
